When the whole abdomen was radiated, the normal kidney was shielded in the posterior t1eld and received approximately one-half the tumor dose. 33 It seems likely that a major reason for the poor survival of children in Group III before 1957 was the failure to adequately irradiate all of the tumor, and, that the change in radiotherapy was responsible, at least in part, for the improved survival in Group III since 1957. The effect of the change in radiotherapy cannot be properly evaluated because only a small number of children were treated with the enlarged field before actinomycin-D was added to the regimen.

Twenty-four per cent had residual tumor in the abdomen after operation (Group Ill), 10% had distant metastases at diagnosis (Group IV), and 3% presented with bilateral tumors (Group V). SURVIVAL Since 1957 the survival has improved in every group (Table III). In Groups I and II, the five-year survival has almost doubled from 40% to 75%; and in Group III, , it soared from 5% to 52%.
