Progressive non-fluent aphasia In this form of lobar degeneration (Snowden et al, 1996), a progressive decline in language occurs in the relative absence of other cognitive deficits, and because of the selectivity of the disorder affected individuals may maintain a high level of functional independence. Speech is non-fluent, effortful and lacking in prosody. Word retrieval difficulties are prominent and there are typically, although not invariably, sound-based (phonemic paraphasic) errors. The ability to repeat, to produce overlearnt series, such as the days of the week, to read aloud, write and spell, are also affected, but spoken and written comprehension at least at the single word level are relatively preserved.

For example, the Carisbrooke service is reluctant to take patients with learning disability because the Learning Disability Service has greater skills in the general management of such individuals even though it has less direct experience of dementia. All new referrals are discussed at weekly team meetings. A decision is taken on whether to accept the referral, and whether to offer a service or provide advice.
